What is Tumescent Liposuction: Information from Your Dallas Dermatologist

The tumescent technique is one of the most significant improvements in liposuction in Dallas . Tumescent liposuction is the use of local anesthesia instead of general anesthesia, and it is so effective that liposuction patients no longer need general anesthesia.

The tumescent technique uses large volumes of a dilute solution of Lidocaine, a local anesthetic, in combination with the drug epinephrine, which temporarily shrinks capillaries. Recently it has been noted that adding a very small amount of Triamcinolone, a cortisone-like anti-inflammatory medication, to each liter of anesthetic solution has resulted in a marked decrease in post-operative soreness.

During tumescent liposuction, fat is removed by suctioning small “tunnels” through the fat. During the healing process, the tunnels collapse and the fibrous strands that connect the skin to the underlying tissue contract. It is this process which assures that the skin will not hang in loose folds after surgery. By ballooning the fatty tissues, the tumescent technique permits more accurate removal of fat with greater assurance that the liposuction instrument will not inadvertently approach too near the under face of the skin, which would cause irregularities. Thus, the tumescent technique helps to minimize the risk of post-surgical irregularities or rippling of the skin.

The tumescent technique dramatically reduces both bleeding during surgery and post-operative bruising and swelling. In fact, there is so little blood loss with tumescent liposuction that patients usually lose more blood during pre-operative laboratory tests than during the actual surgery. Minimal bleeding reduces post-operative recovery time and most patients can return to work and begin light exercise within a day or two after surgery. An elastic support garment is worn for only 10 to 14 days.

What Are the Side Effects of Tumescent Liposuction?

As with any surgical procedure, this procedure is associated with certain expected side-effects such as bruising, swelling and temporary numbness. Although irregularities of the skin are possible following liposuction, this side-effect is minimized by the tumescent technique. As judged by current world-wide experience, tumescent liposuction in Dallas is amazingly safe. Serious complications such as blood clots, infection and allergic reactions are extremely rare.

Post-operative discomfort is also minimized since the local anesthesia remains in the surgically treated areas for approximately 18 hours after surgery. Patients may require only Tylenol for discomfort after surgery.

What Are the Results?

    • About 90% of patients can actually see some improvement in their silhouettes by one week after surgery. However, because of the slow resolution of post-surgical swelling, the ultimate results usually require 9 to 12 months to be achieved. A “touch-up” procedure may be required after another 9 to 12 months, but this is rare.
    • Fat cells that are removed do not grow back, and the patient’s new, more pleasing silhouette is permanent. The changes of silhouette produced are similar to those one would see as the result of dieting to remove specific areas of fat.
    • If the patient gains weight or loses weight, the change tends to be distributed proportionately over the entire body.
    • The degree of skin change after liposuction shrinks.

Tighten Up Your Chest: Advice from Your Dallas Dermatologist on Getting Rid of Man-Boobs

Man-boobs, or gynecomastia, is a condition that causes over-developed or enlarged breasts in men and can occur at any age. This can result from hormonal changes, heredity, obesity or the use of some drugs. Man-boobs can include excess localized fat, excess glandular tissue development, excess breast skin, and can be present in one or both breasts.

There are two types of gynecomastia. Pseudo-gynecomastia is an enlargement of the male breast due to a large amount of fat tissue while keeping a normal amount of glandular breast tissue. Most abnormally large male breasts are the result of excess fat. A small amount of pseudo-gynecomastia occurs in most older men and is common in obese younger men.

True gynecomastia is the enlargement of male breasts due to excessive glandular tissue and is not as common. This can be caused by alcoholism, failure of the testes to produce testosterone, and several medications.

In cases where gynecomastia is primarily caused by excess fat, tumescent liposuction may be used. This involves a large amount of local anesthesia being injected into the fat beneath the skin and causing the area to become firm.

Gynecomastia can cause emotional distress and a lack of self confidence, causing some men to avoid physical activities and intimacy to hide their problem. If you are struggling with gynecomastia and low self-esteem because of it, you may be a candidate for tumescent liposuction in your breast area.

Tumescent liposuction of the male breast will improve physical appearance by removing as much fat as possible while avoiding damage to the skin or muscles. This is one of the most common areas for men to receive liposuction, and men with excessive fat in their chest area can look forward to vast improvement through tumescent liposuction.

If you are tired of feeling embarrassed or uncomfortable with your chest area, contact Dr. Wooming, your Dallas dermatologist, today. We are looking forward to discussing your concerns and finding the best treatment option for your body.

All About Tumescent Liposuction – Dr. George Wooming, Your Dallas Liposuction Specialist

Liposuction, often called “lipo” in popular culture, is a procedure that is done to slim and reshape specific areas of your body by removing excess fat deposits and shaping your body contours and proportion, ultimately boosting your self-image. Dr. George Wooming, your Dallas liposuction specialist, can use liposuction techniques to decrease localized fat deposits in the following areas.

Areas of Focus for Liposuction

    • Thighs
    • Hips and Buttocks
    • Abdomen and Waist
    • Upper Arms
    • Back
    • Inner Knee
    • Chest Area
    • Cheeks, Chin and Neck
    • Calves and Ankles

Dr. George Wooming is a Dallas and Sunnyvale liposuction surgeon who specializes in tumescent liposuction. Tumescent liposuction involves a large volume of dilute solution of local anesthesia to be injected into the fat behind the skin, causing the area of focus to become tumescent, or swollen and firm.

This technique produces long-lasting local anesthesia of the skin and fat, and it eliminates the need for general anesthesia, or IV sedation. By eliminating the risks of general anesthesia and excessive surgical bleeding, the tumescent technique has also eliminated the dangers of the original forms of liposuction. During the procedure, patients are also able to stay awake and experience less pain than patients who have liposuction under general anesthesia.

If you are unhappy with any parts of your body when it comes to extra fat, you may be a candidate for liposuction. The ideal candidate for tumescent liposuction is physically fit and eats a well-balanced diet, but is unable to reduce fatty deposits that are well localized and may seem to involve a genetic susceptibility. Tumescent liposuction, however, is not a treatment for obesity or a substitute for proper diet and exercise.

If you are ready to get rid of unwanted fat and improve your body contours and proportions to enhance your self-confidence, contact Dr. George Wooming, your Dallas  liposuction specialist, today and request an appointment. With two locations in Dallas and Sunnyvale, the staff at Dr. George Wooming’s office will give you the information you need to make the best treatment choice for you. Keep It Together: Advice from Your Dallas Earlobe Splitting Specialist

Are your earrings too heavy? Gauges too thick? Do you have a child who loves to tug on the shiny pieces of jewelry hanging from your earlobes? All of these are reasons you could experience earlobe splitting. Maybe you just had an accident and got your earrings caught on your shirt. Whatever the cause may be, you could be a candidate for earlobe repair. If you have damage to your earlobe, Dr. George Wooming is the Dallas earlobe splitting specialist who can help you get your earlobe looking normal again.

Types of Earlobe Splitting

There are two types of earlobe splitting that can occur. A partial split is where your earlobe is torn from the middle, where your ear piercing would be, to the bottom of your earlobe. A full split is where your earlobe is torn all the way up to the entrance of your ear, past the area where your ear piercing would be.

Prevent Earlobe Splitting

To prevent earlobe splitting, you will want to avoid wearing heavy or thick earrings that feel as if they are tugging on your earlobes. You should also proceed with caution when putting on or removing clothing to avoid catching your earrings on the fabric. Be careful when holding children who might grab onto your earrings thinking they are toys and rip them out of your earlobes. Finally, you can use an earlobe support, or a stick-on disc that will help support the earlobe when wearing heavy earrings.

Treatment

If you have splits in your earlobes, you may be a candidate for an earlobe repair procedure, or an otoplasty. You may also be interested in earlobe repair if you have developed a keloid scar after ear piercing, or an abnormal and itchy scar that can be surgically removed.

Contact us to set up an appointment with your Dallas earlobe splitting specialist and find out the best option for you. Dr. George Wooming, M.D. will be happy to answer any questions you may have at our Dallas dermatology office.

The Wonderful World of Botox and Fillers

Botox is a medically produced drug injected underneath the skin. Botox treats muscular conditions, but it is also used for cosmetic purposes to reduce wrinkles by temporarily paralyzing facial muscles. A botox injection lasts four to six months, then the muscles will slowly return to normal. The wrinkles will become visible again, and the area can be retreated after this period. The more botox treatments, the less wrinkles will show due to the muscles being trained to stay relaxed. Botox injections from Dr. Wooming’s Dallas office can be an alternative to cosmetic surgery, and if retreated often, they can lead to wrinkle-free skin.

The most popular botox areas are the glabella, or the skin between your eyes, the forehead and crow’s feet, however, Dr. Wooming offers several more botox treatment areas at his Dallas office. Other areas where you can receive botox are the bunny lines on the nose when it’s scrunched, the lips, the marionette lines in the corners of the mouth, the soles of the feet, and the hands to prevent hyperhidrosis.

Fillers, or dermal fillers, are soft tissue fillers that add volume to your face without the cost and downtime of surgery. Often referred to as a “liquid facelift,” fillers can be applied to the lips, contours or facial creases and wrinkles. Filler treatments must also be repeated to maintain their effect.

Dr. Wooming also offers several treatment options for fillers at his Dallas location. Dermal fillers can be placed in the cheeks, chin, lips, nasal folds and the bridge of the nose. Fillers can also be applied to the earlobes for plumping and acne scars to make them less visible. Dr. Wooming offers Voluma and Belotero brand fillers. Belotero fillers can be used for crow’s feet and the temples.

Visit Dr. Wooming’s office at his Dallas location for your choice of botox and filler treatments. An alternative to surgery, botox and fillers allow activities to be resumed immediately after the procedure. Contact us to get an assessment of your needs and we will work with you to create a treatment plan that will achieve your desired results.

Fractional CO2

Fractional Laser Procedures

The treatment of choice for acne scars or signs of aging like fine lines, wrinkles, skin laxity and uneven pigmentation is Fractional Laser Skin Rejuvenation.

The Fractional CO2 Laser is a revolutionary new treatment for Skin Rejuvenation that makes it possible for you to have healthy, younger looking skin with minimal post treatment down time. A single treatment can show significant improvement in skin texture and color while softening wrinkles.

How Fractional Laser Treatment Works

The Laser delivers a therapeutic matrix of tiny beams of light energy (each about the size of a hair) to the top and mid layers of the skin with minimal discomfort. This results in….

  • Immediate shrinkage of damaged skin
  • Softening of fine lines and wrinkles
  • Removal of age and sun spots
  • New collagen production* for up to 6 months following treatment

Fractional means that only a fraction of the skin surface is exposed to the laser beam, leaving untreated skin around each treated micro spot. This promotes fast natural healing and a short recovery time. The beneficial effects of the laser beam expands under the skin surface to stimulate new collagen production across the entire area. This innovative technology minimizes discomfort during the treatment, so that for most patients a topical anesthetic is all that is necessary.

Planning For Your Laser Treatment

Prior to Treatment – Your doctor will discuss which areas of your skin will benefit from Fractional CO2 Laser treatments as well as the results you can expect. In most cases, Fractional laser treatment can be safely performed on the face, neck, chest and hands. Usually, a single treatment produces satisfying results. If desired, a second treatment can be scheduled 8 weeks after the first.

The Treatment – The treatment is done in the comfort of your doctor’s office. The areas to be treated are thoroughly washed and the skin is dried prior to treatment. Although some patients do not require a topical anesthetic, your doctor will determine what is best for you and your treatment. During treatment you will feel a warm sensation on the skin. This is to be expected. Treatment time for a full face is approximately 30 minutes.

Post Treatment – You will have a slight burning sensation similar to sunburn, which lasts anywhere from 30 minutes to 3 hours. Your skin will show a fine pinpoint like pattern as a result of therapeutic beams.

Above is a highly magnified image of the skin 48 hours after treatment. This fine pinpoint pattern is typical. In some cases, redness of skin and swelling may occur but generally subside within 24 hours. Apply a fragrance and preservative free moisturizer to the treated area as directed by your doctor.

Considering Botox

Considering BOTOX® Cosmetic?

BOTOX®’s rapid — almost instantaneous – acceptance by patients and the medical community alike is understandable considering previously available options. BOTOX® is generally quick and effective if administered professionally. Unlike other more involved cosmetic procedures like facelifts, BOTOX® is affordable and convenient and is appealing to patients who prefer to “test drive” the results before committing to a permanent solution, since effects last no longer than three to six months.

Despite its unique properties, BOTOX® should be given the same serious consideration as any other drug on the market. BOTOX®’s success is only as good as the doctor who performs the treatment and the patient’s response. Patients must choose their BOTOX® provider carefully, as results are dependent on the mixture concentration level and the doctor’s experience. Dermatologists are medical doctors that specialize in the skin therefore, it is important to make sure your doctor is board certified in Dermatology. BOTOX® is delivered as a freeze- dried powder and must be mixed with saline prior to injection. Without the correct concentration, BOTOX® patients might experience zero effect or droopy eyelids, numbness, and dry mouth. Beware of advertisements offering BOTOX® treatments at an enormously discounted rate. Remember, if the offer seems too good to be true, it probably is. It is crucial to find a doctor you trust.

Prior to its approval for cosmetic purposes, BOTOX® was approved for safe treatment of muscle spasms caused by cerebral palsy, uncontrollable eyelid spasms, and crossed eyes.

On April 15, 2002, Allergan earned FDA approval of BOTOX® Cosmetic for treatment of glabellar lines (the area located between the eyebrows). Since the approval, BOTOX® injections have become the most sought-after cosmetic treatment in clinics nationwide. The popularity of BOTOX® is attributed to its non-surgical nature, low occurrence of side effects, affordability, and effectiveness in reducing moderate to severe brow furrows, horizontal forehead wrinkles, and the tiny lines located at the outer corners of the eyes.

Questions regarding BOTOX®?

If you’re considering BOTOX® facial rejuvenation treatment or other injectable filler procedures, Knox Dermatology offers several options including BOTOX®Radiesse™Restylane® and Fat transfer procedures. Contact us today for a private consultation to determine the right injectable filler procedure for you based on your individual circumstances.

Laser Hair Removal

What is laser hair removal?

Laser hair removal gets rid of unwanted hair by exposing the hair to laser light that desolates the hair follicle. The lasers selectively target dark, coarse hair while leaving the skin unharmed. The pulse of the laser takes only a fraction of a second and treats several hairs at a time in an area about the size of a quarter. Smaller areas such as the upper lip can be treated in a matter of minutes whereas larger areas like the legs and back can take up to an hour.

Preparing for laser hair removal

Before the procedure do not pluck or wax the hair for six weeks. The laser targets the root of the hair and when you pluck and wax the root is temporarily gone.  Also, try to limit your sun exposure for six weeks before surgery as well. The exposure to the sun makes the removal less effective and complications are more likely.

What to expect during laser hair removal

Before the procedure begins you will be prepped for the treatment. The unwanted hair will be trimmed down to a few millimeters above the surface of the skin. The laser itself will be adjusted to your skin color and hair type. The doctor will use a cold gel or a cooling device to protect the outer layers of the skin – this will also help the laser penetrate the skin.

After the procedure is complete, ice packs and anti-inflammatory lotions and gels will help with any discomfort. You will be able to come back in for the next treatment in about six weeks and can have as many sessions until the hair stops growing.

Benefits of laser hair removal

  1. Less shaving. After about your fifth treatment your hair growth has reduced 70-90% which can reduce shaving to approximately every six months. This is sometimes the equivalent of one full week of your year.
  2. Less ingrown hairs. The risk of getting an ingrown hair is reduced by 70-90% because you are shaving less.
  3. Softer and smoother skin. Even after the first treatment the hair is not as thick which means your skin is smoother and softer. When waxing, hair reappears after 2-3 days.
  4. Fewer blemishes. Many times after shaving people will develop skin rashes which can cause post inflammatory hyper-pigmentation. This can leave scars and blemishes on the skin, but since you have to shave less often the risk of this occurring is reduced.

What is Sclerotherapy Used For?

Varicose veins and spider veins

Varicose veins are enlarged veins that can be blue, red, or flesh-colored often found on the thighs, backs of the calves, or the inside of the leg. They often look like cords and appear twisted and bulging appearing swollen and raised above the surface of the skin.

Spider veins are like varicose veins but smaller and are closer to the surface of the skin. They are red or blue and can look like tree branches or spider webs with their short, jagged lines. Spider veins can be found on the legs and face and can cover either a very small or very large area of skin.

Weak or damaged valves in the veins can cause varicose veins. The heart pumps blood filled with oxygen and nutrients to the whole body through the arteries. Veins then carry the blood from the body back to the heart. As the leg muscles squeeze, they push blood back to the heart from the lower body against the flow of gravity. Veins have valves that act as one-way flaps to prevent blood from flowing backwards as it moves up the legs. If the valves become weak, blood can leak back into the veins and collect there. When backed-up blood makes the veins bigger, they can become varicose.

Varicose veins can often be seen on the skin. Some other common symptoms of varicose veins in the legs include:

  • Aching pain that may get worse after sitting or standing for a long time
  • Throbbing or cramping
  • Heaviness
  • Swelling
  • Rash that’s itchy or irritated
  • Darkening of the skin (in severe cases)
  • Restless legs

What is Sclerotherapy used for?

Sclerotherapy is the most common treatment for both spider veins and varicose veins. The dermatologist uses a needle to inject a liquid chemical into the vein. The chemical causes the vein walls to swell, stick together, and seal shut. This stops the flow of blood, and the vein turns into scar tissue. In a few weeks, the vein should fade. The patient can return to normal activity right after treatment. This treatment does not require anesthesia and can be done in a dermatologist’s office. Contact our Dallas Dermatology office to set up a consultation and assessment of treatment for varicose veins.

Dermal Fillers – A Facelift Alternative

Looking for something to rejuvenate the lower face?

Dermal fillers have been called “liquid facelifts” because they offer many of the benefits of a surgical facelift without the downtime. These soft tissue fillers can restore a youthful fullness to your face, plump lips, enhance shallow contours or soften facial creases and wrinkles.

Although they cannot help with excess sagging skin, dermal fillers can add more volume and provide immediate results at a lower cost than surgery. These treatments are not permanent, and they must be repeated to maintain that fresh, full new look.

Dermal fillers – a facelift alternative

Some people opt to have a liquid facelift, delaying the time when a surgical facelift is more appropriate. It’s a great way to see some impressive changes and improvements in the facial appearance, such as the reduction of forehead wrinkles, until one is ready for a permanent surgical option.

The dermatologist will listen to the patient’s desired results and then evaluate the facial appearance and skin tone, examining the areas of the face to be augmented with a cosmetic dermal filler. The dermatologist will then mark strategic points that will be used as guides to the appropriate injection sites for the dermal filler and the injection sites will be cleansed with an antibacterial agent. Then a topical anesthetic will be used to numb the area. The actual injections will take just a few minutes total, and just seconds per site. The marks will be washed away and an ice pack will be given to reduce any minor and temporary discomfort.

There is no downtime with dermal fillers so most activities can be resumed right away.  Most patients experience swelling or a bit of bruising, which should fade within a few days. The treated area should not be rubbed so that the filler material remains undisturbed.

It’s important to realize that dermal fillers are not permanent and that even the semi-permanent varieties eventually require re-treatments. The way the face continues to age and how the body absorbs fillers will determine the timing of repeat treatments. If not re-treated, the appearance will return to its original condition, meaning that wrinkles and scars will return, and plumped lips will lose volume.

If you are thinking about getting a facelift but don’t like the idea of surgery, dermal fillers are a great alternative and are offered at George Wooming, M.D.’s Dallas office. Contact us to get an assessment of your needs and a recommendation for one or a combination of treatments to achieve your desired results.
